Add a New Form<\/strong><\/span><\/p>\n

After clicking “Forms<\/strong>” in the left menu column, in the page that displays (see above), click \u201cAdd new<\/strong>\u201c. <\/span><\/p>\n

\"\" \"\"<\/p>\n

Define the following information:<\/span><\/p>\n

    \n
  1. Title<\/strong>\u00a0which will be displayed on the dashboard and summary page.<\/span><\/li>\n
  2. Short description\u00a0<\/strong>which is displayed in the summary page.<\/span><\/li>\n
  3. Full description<\/strong> is the form’s background information that is displayed when user clicks to view the contents in the form.<\/span><\/li>\n
  4. We strongly recommend that you upload a thumbnail picture<\/strong> for the form. This is displayed in the summary page.<\/span><\/li>\n
  5. Groups <\/strong>is to assign this form under one or more Groups.<\/span><\/li>\n
  6. Start<\/strong>\u00a0and\u00a0End date\u00a0<\/strong>of the form to be displayed. Take note that status has to be \u201cActive\u201d for the form to be effective.<\/span><\/li>\n
  7. Status<\/strong> of the form. This can be:<\/span>\n
      \n
    1. \u201cDraft\u201d which is only visible for administrator\u2019s preview and not user\u2019s,<\/span><\/li>\n
    2. \u201cActive\u201d where article appears between the start\/end date, or<\/span><\/li>\n
    3. \u201cInactive\u201d which does not display the form.<\/span><\/li>\n<\/ol>\n<\/li>\n
    4. Limited to stores <\/strong>is an optional setting to restrict the form to a selected store, when there is more than one store.<\/span><\/li>\n
    5. Company<\/strong> is to assign the Company whose users can view this form, when there is more than one company.<\/span><\/li>\n
    6. BU under company<\/strong> is to assign the business units (BU) within the Company whose users can view this form. This provides more detailed user targeting for the form.<\/span><\/li>\n
    7. Sub BUs to include<\/strong> is a flag to further target users for this form. This can be:<\/span>\n
        \n
      1. \u201cInclude Sub BUs within the company only\u201d is to exclude all sub BUs that do not belong to this company,<\/span><\/li>\n
      2. \u201cInclude Sub BUs in all levels below\u201d is to include all sub BUs within this company’s hierarchy, plus all sub BUs of other companies that are in the same hierarchy,<\/span><\/li>\n
      3. \u201cNone\u201d is to exclude all sub BUs of this company.<\/span><\/li>\n<\/ol>\n<\/li>\n
      4. Grade<\/strong> is to target the form to users based on grade, for example job grade.<\/span><\/li>\n
      5. Category<\/strong> is to target the form to users based on category, for example category of users.<\/span><\/li>\n
      6. Editor BU<\/strong> is for information only and shows the business unit (BU) that created the form.<\/span><\/li>\n
      7. In\u00a0Managed by<\/strong>, the form can be managed by 1) Editor or Publish BU, 2) Editor BU only or 3) Publish BU only. We suggest to use the default, which is option 1.<\/span><\/li>\n
      8. Enable notifications?<\/strong> is a flag to enable notifications for this form.<\/span><\/li>\n
      9. Additional Publish BU<\/strong> is to assign additional Companies, BUs, Sub BUs, Grade and Category, to this form.<\/span><\/li>\n<\/ol>\n

        Save the form first before adding Questions<\/strong>. Click \u201cSave and Continue Edit<\/strong>\u201d to remain within the form. Then proceed to the Questions<\/strong>\u00a0tab.<\/span><\/p>\n

        Click “Add a new question<\/strong>” and define the following information:<\/span><\/span><\/p>\n

          \n
        1. Question<\/strong> which is the question displayed in the form.<\/span><\/li>\n
        2. Is required <\/strong>is to flag mandatory question(s) in the form, which users must respond to.<\/span><\/li>\n
        3. Type of answer<\/strong> are the format options for answering this question. This can be:<\/span>\n
            \n
          1. “Drop down” display a drop down list of answer values where any one value only is selectable,<\/span><\/li>\n
          2. “Multiple choice” display a range of answer values where any one value only is selectable,<\/span><\/li>\n
          3. “Checkboxes” display a range of answer values where more than one value is selectable,<\/span><\/li>\n
          4. “Short answer” display a text box with a defined “Minimum length<\/strong>” and “Maximum length<\/strong>” for a short answer<\/span><\/li>\n
          5. “Paragraph” display a text box with a defined “Minimum length<\/strong>” and “Maximum length<\/strong>” for a long answer<\/span><\/li>\n
          6. “Date” display a pop-up date selector for the date answer<\/span><\/li>\n
          7. “File upload” display a pop-up file selector with a defined “Allowed file extension<\/strong>” and “Max file size (<10240 KB)<\/strong>“<\/span><\/li>\n
          8. “Color squares” display a range of color choices where any one value only, is selectable. For example, select a color for a gift T-shirt.<\/span><\/li>\n
          9. “Image squares” display a range of image choices where any one value only, is selectable. For example, select a gift mouse design.<\/span><\/li>\n
          10. “Read only checkboxes” display a range of user defined checkboxed read only text content.<\/span><\/li>\n
          11. “Title” display a read only user defined text content.<\/span><\/li>\n<\/ol>\n<\/li>\n
          12. Default value<\/strong> is to display a default answer value for the question.<\/span><\/li>\n
          13. Input the\u00a0Display order\u00a0<\/strong>of the Questions with the lowest number on top<\/span>.<\/span><\/li>\n
          14. Values<\/strong> is for keying the range of answer values for the Question. Click on the “Values” title and then “Add a new value” to start the input screen.<\/span>\n
              \n
            1. Name<\/strong> is the displayed description for the answer value.<\/span><\/li>\n
            2. Is Pre-selected<\/strong> is to flag an answer value to display as pre-selected.<\/span><\/li>\n
            3. Input the\u00a0Display order\u00a0<\/strong>of the answers with the lowest number on top.<\/span><\/li>\n<\/ol>\n<\/li>\n
            4. Condition<\/strong> is to set this Question as hidden. This Question is displayed only when another Question, defined below, is responded to at a pre-defined answer.<\/span>\n
                \n
              1. Enable<\/strong> condition is to enable a condition to apply to this Question.<\/span><\/li>\n
              2. Question<\/strong> is to select a prior Question and its pre-defined answer, that triggers the display of this Question.<\/span><\/li>\n<\/ol>\n<\/li>\n<\/ol>\n

                Refer to screenshot titled “Edit form – Application to be part of Ren News Editorial Team for 2022”. Question 5 is conditional upon question 4. Question 5 is hidden and the user can respond to it only after question 4 has been responded to with the pre-defined answer.<\/span><\/p>\n

                Link a Form from a news article<\/span><\/strong>
                \nWhen a cover page is needed to go with a form, it is done by linking the form to an article. The below screenshot is from the bottom section of the “Add new article” page.<\/span><\/p>\n

                1. Link to?<\/strong> is to flag what this article is to be linked to. Select “Form” to link to a form.<\/li>\n
                2. Link label<\/strong> is the text to display inside the article for a clickable link.<\/li>\n
                3. Show which forms?<\/strong> is to define what list of forms is available for selection in the below drop down.<\/li>\n
                4. Form<\/strong> is the drop down list of forms for selection and linking<\/li>\n<\/ol>\n

                  In the published article, the link appears at the bottom of the article’s page as shown below.<\/p>\n
